"This Week" Door Prize Drawings and Others
- Eligibility

All Peryam & Kroll Research reward Promotions are open only to legal
residents of the United States, living in Illinois/Indiana, California, Texas,
or New York/New Jersey/Connecticut 8 years of age or older. "P&K Tester
Rewards" Promotions are void in any other state. Promotions are scheduled on a
periodic basis and announced to P&K Research Testers during the study
screening process. P&K Research Testers are automatically entered to win
game promotions after they screen-qualify and complete a research study. Peryam
& Kroll Testers who are denied participation due to schedule
incompatibility, screen failure, re-screen failure, non-compliance with study
or facility requirements, absence or late arrival at their appointed time will
not be entered to win. The prize check will be issued in the winner’s name. If
an individual under 18 years of age is selected as the winner, his or her
parent or legal guardian is required to give written authorization to release
prize. Employees, officers and directors of Peryam & Kroll Research
Corporation, Peryam & Kroll Healthcare Research, their legal
representatives, and each of their affiliates, parents, subsidiaries,
advertising, promotional, fulfillment and marketing agencies, their immediate
families (and those living in their same households) are not eligible to
participate or claim prizes in these promotions.

By entering these Promotions, participants agree to release, discharge and hold
harmless Peryam & Kroll Research Corporation, their legal representatives,
their affiliates, subsidiaries, retailers, agencies and their respective
officers, directors, employees and agents, from any damages which may arise out
of participation in these Promotions, or out of the acceptance, use, misuse or
possession of prizes. By participating, you agree to these Official Contest
Rules and to the decisions of the judges, which are final and binding in all
respects. The Official Contest Rules will be posted at the office of Peryam
& Kroll Research Corporation, 6323 N. Avondale #211, Chicago, IL 60631
throughout the contest and are available online HERE
or upon request by sending a self-addressed stamped envelope and request to
that address. Void where prohibited by law, rule or regulation. All federal,

- How to Enter

When scheduled to participate in a study at Peryam & Kroll Research Corp.,
show up and complete your study and you will be automatically entered to win.
Testers will be notified during the screening call that a promotion is running
and that they will be entered to win upon completion of their study. They are
eligible to receive "This Week" awards for the week, Monday-Sunday, in which
they completed their study. For multiple day studies, the final day of testing
determines the week of eligibility. For in-home testing, the date of final
call-in or product drop-off, whichever is later, determines the week of
eligibility. Additional prizes may be awarded on a same-study, same-day or
same-session basis. Only one win per entry. Multiple entries are not permitted
and will count as one entry.
- How to Play for "This Week" Tester Rewards

Peryam & Kroll Research Corp. will draw names randomly from all persons who
have been automatically entered after qualifying and completing a study during
that week, Monday-Sunday. Three prizes of $100, $150, and $250 will be awarded
to three "This Week" winners. No purchase is necessary to win and winners need
not be present. A check will be mailed to each winner.

If included in the promotion, additional drawings from all persons who have
been automatically entered for a specific study or specific day of testing may
be awarded in the same manner.

If included in the promotion, additional drawings from all persons who have
been automatically entered for the session in which they participated may be
awarded in the same manner. However, winner needs to be present for session
drawing. Cash or check will be awarded to winner at the time of drawing.

Chances of winning any prize depend on the number of entries received. There is
a maximum of three "This Week" winners. Odds of winning a Week-of prize are
subject to change based on number of respondents age 8+ needed per week, and
are usually no greater than 1 in 1700. Odds are updated and posted regularly on
our website HERE.

- Tax Information

All taxes and fees on prizes won are the sole responsibility of the winner.
P&K Research reserves the right to withhold taxes from each winning prize
as appropriate.
- Unclaimed Prizes will not be Awarded

A check will be mailed to each winner within 3 days of drawing. All reasonable
actions will be taken to notify the winner, verify their mailing address and to
deliver their prize. Return of any prize and / prize notification as
undeliverable may result in disqualification within 30 days of its return. In
the event that a returned prize is held for the winner to pick up, winner will
be required to provide picture proof of identification, which may include a
driver’s license, passport, a voting card or similar government issued
identification.
- Publication

Acceptance of any prize constitutes permission for P&K Research, its
affiliates, subsidiaries, advertising and promotional agencies to use winner’s
name and likenessrsquo; for advertising and promotional purposes without
additional compensation unless prohibited by law. Each winner further grants to
P&K Research the right to use and publish their identity and home state
online and in print, or any other media, in connection with this Promotion.
